Create an Intervention Plan

Answer the following questions as they pertain to Carla Washburn.

Step 1: Goals and Needs

  1. Which one of Carla Washburn’s goals seems most important to focus on now? How does Mrs. Washburn signal her priority for beginning the work?
  2. What resources will be required? Include both the formal and informal resources you identified earlier, as well as additional resources that do not appear on the town map.

Step 2: Client Tasks

  1. What will Ms. Washburn need to do to accomplish the goal(s) you have defined?
  2. How can neighbors/friends, such as Mrs. Minor, support Ms. Washburn? What actions would be mutually beneficial? Do you see any possibility of a mutual exchange of resources or energies?
  3. How can Pastor Smith and the church help Ms. Washburn meet her goals? Are there any actions that would be mutually beneficial?
  4. How might the Jackson or Johnson families assist Ms. Washburn, and be helped in the process?

Step 3: Social Worker Tasks

  1. What can YOU do that the client cannot? How will you contract with Mrs. Washburn to agree on your role?

Step 4: Timeline

  1. What time factors are involved in accomplishing this goal? How long do you anticipate it will take? Does something else have to happen? Is this a goal that can be accomplished by a certain deadline?

Step 5: Coalitions

  1. Who or what else is critical to accomplishing this goal? What role do they play in the client’s life? What is the likely response, and if it is negative, what can you and the client do to overcome this barrier?

My Intervene Tasks

Task 1

Begin your intervention by defining Carla Washburn’s goals and needs in the order of their importance and the speed with which they can be accomplished (it is important to your work with Ms. Washburn that you meet with at least one success early in the process. Empowerment theory suggests that this will propel her to accomplish the more difficult goals)

Task 2

Develop a cohesive plan of action for this client that will factor in the tasks the client will have to accomplish as well as those the social worker will have to complete in order to reach the client’s goals.

Task 3

A critical piece of your work is the mobilization of people, places, and resources that will help Ms. Washburn reach the goals you have defined together. Using the ecomap tool and symbols, diagram these new coalitions.
